Frequently Asked Questions

You can contact your pediatrician’s office, your high school, your previous college, etc. and ask if they have your records. 

If you cannot obtain records, please make arrangements to receive the vaccines again OR provide serology results that show immunity.

Health cleared means you have completed and submitted the Required Immunization documents to Med+Proctor.

Exception: Students enrolled in Online Programs Only are not required to meet immunization requirements.

New entrants, transfer students or former returning students who have been accepted to 91勛圖厙, are automatically placed on "not health cleared hold" until you satisfy the immunization requirements. Once you have submitted the “Required Immunization” documents to Med+Proctor your health clearance hold will be released.

Once your documents have been received, you will receive a secure message through the indicating you are compliant.  If you are missing or need to update any of the required immunizations, the secure message will indicate that you are non-compliant and will include instructions.  You can monitor your status on the portal.  Please allow 48 – 72 hours for processing.

Acceptable documents include:

  • a document signed by a health care provider that indicates the names of vaccines and the dates you received them, OR
  • a copy of your immunization records from your previous school on the school’s letterhead
  • lab results showing your immunity.
  • State computer generated printouts of previous vaccines will be accepted without a physician signature.

Louisiana law ) requires proof of dates of immunization against measles, mumps, rubella, and tetanus-diphtheria for all first-time students born on or after January 1, 1957, and for reentering students (born on or after January 1, 1957) who have been out of school for one semester or longer. The following guidelines are presented for the purpose of meeting the established recommendations for control of vaccine-preventable diseases, as recommended by the American Academy of Pediatrics (AAP), the Advisory Committee on Immunization Practices to the United States Public Health Service (ACIP), and the American College Health Association (ACHA). In addition, current Louisiana legislation now mandates meningitis vaccination for persons being admitted to a postsecondary education institution for the Fall 2006 semester and thereafter.

Yes, ​Measles, Mumps, Rubella (MMR) vaccines are good for life, the Tetanus-Diphtheria (Td/Tdap) once every 10 years, Meningitis one (1) dose at age 16 years or older.

The State of Louisiana requires anyone attending school in Louisiana to submit this documentation in order to attend school in Louisiana. If you are unable to obtain a copy from your previous school or family physician you will have to retake some and/or provide titers for the other required immunizations. (MMR's, Tetanus, Meningitis)

You may obtain immunization records at any time. The fastest, easiest way is to log into the , click on My Profile and Immun. History print your own copy or if immunizations were submitted through Med+Proctor you may log into your account () and request a copy.

If you are unable to access the records online, you may visit Student Health Services in person or fax the Medical Release form to Student Health Services @ (504)520-7962. All request will be fulfilled within 72 hours.

Students who are not immunized for medical or personal reasons must complete and submit an Immunization Exemption form to Med+Proctor.

In the event of an outbreak: any student who has an exemption on file will be asked to leave the university until the outbreak is over or until they submit proof of immunization.

The University does not refund any tuition or fees to students who are unable to attend or complete classes or coursework due to exclusion from campus for lack of immunizations. Further, students who receive exemptions are required to release and hold harmless the University and its trustees, officers, agents, servants and employees from any liability incurred or sustained by the University which arises out of the University's consideration of, or ultimate decision to grant, an exemption request.

NOTE: Students who request to reside on-campus in 91勛圖厙 main campus or any of the satellite campus residential halls must complete and submit all required immunizations before move in day.  No Exceptions

Students with medical contraindications or religious affiliation that prevents them from getting vaccinations must submit a completed Request for Medical/Nonmedical Immunization Exemption Form to Med+Proctor.

Yes, all students currently enrolled in the university are eligible to be seen in Student Health Services clinic regardless if they are covered by their own health insurance or by the student health insurance plan.

No, all visits are confidential and are not part of your academic record. Information in the medical record may only be shared if you sign an authorization to disclose protected health information.

Insurance information is obtained to provide to off-campus ancillary providers if a student is referred for x-rays, labs, etc.

No, there is one pharmacy within walking distance and several other pharmacies near the campus

that accept most prescription plans.

Yes, there is a fee charged for copies of medical records.  Fees are listed on the Authorization for Release of Health Information form.  Payment information may be submitted along with the Authorization for Release of Health Information form or you may contact the Cashier's Office @ (504)520-5226 to submit payment.

Students wishing to obtain a copy of their medical records must complete an Authorization for Release of Health Information form and fax completed form to (504)520-7962 or mail to: 1 Drexel Drive, Box 36, New Orleans, La. 70125.

All medical records are considered confidential. If submitting a copy of your medical records to us, please make sure to put your student ID number on each page of your document(s) and you may mail, hand deliver, fax them to: Fax# (504)520-7962 or upload through the Patient Portal.
